Patient Data and Privacy Notice

Below you will find the Data and Privacy Notice you will be provided with when you become a patient of Considerate Foot Care.  This is supplied to you when you complete the Patient Registration, Medical History and Consent to Treat forms prior to your first treatment.

In order to deliver healthcare treatment, we will need to collect and process personal data. It is a legal requirement that this data is stored for a number of years (which can vary depending upon the persons age and circumstances but will be for a minimum of 7 years).

Data is held on our password-protected computers/phones, in our lockable filing cabinets, and/or on cloud-based secure internet servers, including via practice management software. Your personal (medical) information will only be used to enable us to give you appropriate treatment and advice, including for communication with other health care providers. It will only be accessed by Considerate Foot Care staff and associates.

You can access all of your personal data that this organisation stores by request. You can write to us to request or seek further information.

On occasions as appropriate, we ask for permission to use your data to send to another healthcare practitioner, this will only be done with your consent.

Your contact information will only be used for the purposes of contacting you if necessary. This may be to remind you of an appointment, to follow up treatment, or to re-arrange a scheduled appointment. With your permission (that can be changed at any time by yourself), we may use this information to send you relevant information, including marketing / promotional material. We will not sell or share your contact information with anyone else.

Any other use of your personal data will only occur with your informed consent as confirmed by your signed declarations.
